A Place At Home has partnered with the virtual nursing platform PocketRN to enhance dementia care through the Centres for Medicare & Medicaid Services (CMS) GUIDE Model in the US.
The CMS Guide Model is a national initiative aimed at improving care for individuals living with dementia.
This collaboration will see A Place At Home franchise owners providing in-home respite care for families enrolled in the GUIDE programme via PocketRN.
Eligible patients and caregivers will benefit from Medicare-covered respite services delivered by A Place At Home, a senior home care provider franchise.
They will also benefit from 24/7 virtual nursing support and ongoing caregiver management from PocketRN.
The programme delivers four-hour increments of respite care, which totals up to 72 hours yearly. This initiative aims to bring meaningful support directly into homes.

Local franchise owners will refer clients with diagnosis or suspected diagnosis of dementia enrolled in Medicare Parts A and B to PocketRN, which will conduct assessments in line with CMS requirements.
Once approved, A Place At Home teams will perform home safety evaluations and commence care delivery.

The initiative is also expected to ease pressure during post-hospital transitions. Training and bi-weekly webinars will prepare franchise owners for operational delivery, while after-hours nursing calls will be managed directly by PocketRN.
